Step #1:  Remove the screw covers by using a thumb tack, then remove the screws by using a small phillips-head screwdriver.

Step #2:  The picture below will guide you in removing the faceplate, the circled areas show you where the faceplate clips onto the phone.  Focus on the circled areas when removing the faceplate.

Step #3:  By using the same phillips-head screwdriver, remove the screw inside the YELLOW circle.  Then transfer the camera lenses metal piece over to the new faceplate, secure the screw in the RED circle.

Step #4:  Snap the new faceplate onto the phone, secure the screws, put back the screw covers, and you are all done.
